Post-infectious acute glomerulonephritis is the most common acute glomerulonephritis. A typical case would be of an eight-year-old boy who is brought to the pediatrician because of swelling of the eyes and legs, tiredness and headaches for two days after completing a 10-day course of penicillin for a strep throat infection. The urine is noticed to be Coca-Cola colored and the mother, for instance, can be the one saying these words. On physical exam the blood pressure will be 150/85, high, edema of the lower extremities, lungs are clear to auscultation however on an x-ray there is increased vascular markings and an enlarged heart. The urinalysis reveals 3+ proteinuria and too numerous to count red blood cells, and RBC casts. So this is a typical story of acute glomerulonephritis with these features highlighted. It’s an abrupt onset, about a week to two weeks after a pharyngitis or a 2-3, even 4 weeks after a skin infection. We mentioned already, the very typical Coca-Cola colored, or tea colored, urine. The basic volume status of hypervolemia manifested by edema and hypertension. The hypertension can cause encephalopathy, headaches, confusion and even convulsions. And it doesn’t have to be malignant hypertension in measuring. It can be high blood pressure for this kid, which all of a sudden went up abruptly. There is oliguria, and therefore circulatory congestion with pulmonary edema, dyspnea. The things that are found on the x-ray are the increased markings and the big heart.
