This is a brief history of a typical patient with pancreatitis. A 58-year-old man, acute onset of midepigastric pain that is sharp, penetrating through to his back and associated with an elevated amylase. At that point in time - although we wouldn’t do these imaging studies - I would expect the pancreas is as pristine and beautiful as we see on this CT scan. However, what the clinician hopes for at that point is that that patient will fall into the 80% of patients with acute pancreatitis, that in fact have self-limiting disease. It resolves in a few days and it has really a minimal morbidity and mortality. What we worry about is that 20% of patients who have a very substantial morbidity and mortality, 15-30%. In a few days that pancreas may well go on to look like this with edema, areas of necrosis. It may be even worse with a broadly edematous gland. Here there’s a disruption in the duct and leakage of contrast, and by the time that happens we try to convince our surgeons to take out something that is as ugly as this.
